 PENNSYLVANIA CRIME STOPPERS IS OFFERING A CASH REWARD OF UP TO $2,000.00 THAT LEADS TO THE ARREST OF JERRY LEE POLACHEK. 
The Pennsylvania State Police, Troop B, Waynesburg continue to search Jerry Lee Polachek.
 
On November 1, 2005, while committing a daylight burglary of a residence in Dunkard Township, Greene County, Pennsylvania, Mr. Polachek was confronted by the homeowner.  Mr. Polachek fled the scene on foot.  A positive identification of Jerry Polachek was established and an arrest warrant was issued. All items removed from the house were recovered in surrounding areas of the home.   
 
The last known address for Jerry Polachek was 830-A Forbes Road, Monroeville, Allegheny, Pennsylvania.  He is also known to frequent the Turtle Creek, Verona, Swissvale, and Edgewood areas; all located in Allegheny County.
